A camera writes EXIF tags for the capture date and time, the GPS position when location is on, the make and model of the device, the lens, exposure settings and the orientation. Editing tools add XMP tags that record the software and, sometimes, the history of changes.
Not every field is always present. Phones let you turn location off, and many apps remove tags on export. So a photo with rich metadata and one with none can both be genuine.
EXIF holds the technical capture data written by the device, such as date, GPS and camera settings.
XMP is an Adobe format for editable metadata, often used to store editing history and rights information.
IPTC carries descriptive fields like caption, author and keywords, mostly used by newsrooms and stock libraries.
EXIF tags are trivial to change. Anyone can edit the capture date, move the GPS coordinates or wipe every field with a free tool in seconds. A screenshot or a trip through a messaging app removes them entirely.
So metadata answers what a file claims about itself, not what actually happened. It is a useful hint, never a guarantee.
